CBTC E-Consent Disclosure


E-Consent Disclosure Regarding Conducting Business Electronically, Receiving Electronic Notices and Disclosures, and Signing Documents Electronically

Please read the following information, by proceeding forward and signing this document you are agreeing that you have reviewed the following consumer disclosure information and consent to transact business using electronic communications, to receive notices and disclosures electronically, and to utilize electronic signatures in lieu of using paper documents. This electronic signature service is provided on behalf of Commercial Bank and Trust, who is sending electronic documents, notices, disclosures or requesting electronic signatures to you.

You are not required to receive notices and disclosures or sign documents electronically. If you prefer not to do so, you may request to receive paper copies and withdraw your consent at any time as described below.

In this ESIGN Consent:

"We," "us," "our" and "CBTC" means Commercial Bank and Trust Company.

"You" and "your" means the person giving this ESIGN Consent.

"Communications" means each disclosure, notice, agreement, fee schedule, record, document, and other information we provide to you, or that you sign, submit, or agree to at our request.

"CBTC Product" means each and every account, product, or service we offer that you apply for, own, use, administer or access, either now or in the future.

Paper Copies

You are not required to receive notices or disclosures or sign documents electronically and may request paper copies of documents or disclosures if you prefer to do so. You also have the ability to download and print any open or signed documents sent to you through the electronic signature service using the PDF and Print icons. Adobe may also email you a PDF copy of all agreements you sign using the electronic signature service. If you wish to receive paper copies in lieu of electronic documents, you may close this web browser and contact CBTC by telephone at 888-518-7053. CBTC may apply a charge for requesting paper copies unless charging a fee is prohibited by applicable law.

Hardware and Software Requirements

Use of the Service requires a standards-compliant web-browser which supports the HTTPS protocol, HTML, and cookies. Viewing PDF documents requires additional software such as Adobe Reader or similar.

Withdrawal of Consent

You may withdraw your consent to receive electronic documents, notices or disclosures at any time. In order to withdraw consent you must notify the CBTC that you wish to withdraw consent and to provide your future documents, notices, and disclosures in paper format. After withdrawing consent if at any point in the future you proceed forward and utilize the electronic signature system you are once again consenting to receive notices, disclosures, or documents electronically. Any withdrawal of your consent to receive electronic

Communications will be effective only after we have a reasonable period of time to process your withdrawal.

You may withdraw consent to receive electronic notices and disclosures and optionally electronic signatures by contacting CBTC at 888-518-7053.

Depending on the specific CBTC Product, if you withdraw consent, we may charge higher or additional fees for that product or for services related to it.

Please refer to the applicable agreement for any fee that may apply.

Scope of Consent

You agree to receive electronic notices, disclosures, and electronic signature documents with all related and identified documents and disclosures provided over the course of your relationship with CBTC. You may at any point withdraw your consent by contacting CBTC at

We may always, in our sole discretion, provide you with any Communication via paper, even if you have chosen to receive it electronically.

Sometimes the law, or our agreement with you, requires you to give us a written notice. You must still provide these notices to us on paper, unless we specifically tell you in another Communication how you may deliver that notice to us electronically.

There are certain Communications that by law we are not permitted to deliver to you electronically, even with your consent. So long as required by law, we will continue to deliver those Communications to you in writing. However, if the law changes in the future and permits any of those Communications to be delivered as Electronic Records, this ESIGN Consent will automatically cover those Communications as well.

Updating Your Contact Information

It is your responsibility to provide us with true, accurate and complete e-mail address, contact, and other information related to your account(s). You must notify us of any change in your email address or other contact information. We may treat your provision of an invalid e-mail address or the subsequent malfunction of a previously valid address as a withdrawal of your consent to receive electronic Communications.
